Online Application Process
There are only 4 Easy Steps to complete the online application process:
- Click Apply Now then select the Campus and Department you wish to enroll.
- Fill out the form completely and choose ‘submit’.
- Open your E-mail. An auto-generated form will be sent to the e-mail address you provided. (If you have not received the form within 3 working days, you may report it directly to lams.admissions@gmail.com for assistance.)
- Print the form and collate the following documents (for New Students only) needed on your scheduled On-Campus Enrollment:
- Original Report Card (duly signed by the Principal)
- Original PSA Birth Certificate
- 2×2 ID Pictures (4 pcs.)
- Good Moral Certificate
- Certificate of Honors Received
- Photocopy of Certificate of Completion / Diploma
- SF-10

Reminders Before Filling out the Form:
- There are Two Enrollment Forms in this portal:
- The New Student Application Form (for Transferees and Incoming Preschool Students), and the
- Old Student Registration Form (for Old Students only);
- Make sure that you provide an “active” e-mail address; and
- Prepare for the Learner Reference Number (LRN). This is a required field in the enrollment form (except for incoming Preschool students). Please refer to the student’s ID, report card and/or diploma for the copy of his/her LRN, or contact your child’s adviser.
