Email Application Process

Download the application form here.

1. Fill out the application form and attach a 2×2 photo.
2. Scan/ take a clear shot of the filled-out form, save as .pdf file or .jpeg file and send to the official Gmail address of the particular department you are enrolling your child/children, which are as follows:
   
   • Pre-Elementary – preelem@mylams.edu.ph
   • Elementary – elem@mylams.edu.ph
   • Junior High School – junior.hs@mylams.edu.ph
     
3. Wait for acknowledgement from the Admissions Office regarding the receipt of your registration form and for further details regarding your child’s registration to LAMS.
